Tha for inquiring about the services of 

Family Outreach Counseling Center, Inc.(FOCC)


When you book your appointment, please email  a copy of your

insurance card, front and back, so that FOCC can verify your EAP eligibility. Email to : (familyoutreachcounselingcenter@gmail.com)



Check with your employers insurance benefits or the HR department to inquiry about EAP Benefits.


Once you have confirmed that your insurance has EAP Benefits, call your EAP provider to inquiry about the number of visits.


Contact your EAP provider to confirm the number of visits/sessions per year you are eligible for.


When you contact your EAP provider, ask them if a referral is required from your PCP.


It is important that you provide FOCC with the following: EAP Insurance provider (i.e. Aetna, Blue Cross, United Health Care);  EAP Authorization Number; the number of sessions you are approved for.  All of this information should be provided to you when you contact your EAP provider.
